Complete the attached form to highlight your leadership, academics, and service experiences during your high school years, describing your leadership positions, academic honors, IB/AP/dual enrollment courses, and approximate amount of community service hours for each service involvement activity.

Choose one (1) of the following two (2); 500 word minimum.

Describe a community service project or experience that has required you to sustain your commitment over an extended period of time. Please include: a) the specific leadership actions that you experienced in this project and b) explain your motivation for of your commitment.

In order to demonstrate extended commitment to a task or achievement in a leadership position, you may submit a portfolio regarding your project. The portfolio may include an audio or videotape, promotional materials including press releases, flyers, or photos, etc. Please include a detailed written description of the project. Essay

Letter of Recommendation
The letter of recommentdation should be from a high school administrator (principal, vice principal, guidance counselor, student council advisor, program or organization advisor) or a community advisor who can attest to your leadership experience.
